Medical Clearance

In order to try out for, or participate in, a varsity sport at the University of Chicago, every student must complete the required athletic training forms and complete a medical review on campus. The schedule for medical clearance will be communicated via sports coaches and posted in the Ratner Athletics Center and Henry Crown Field House.

All forms and medical documentation must be turned into the athletic trainer's office prior to the student being allowed to try out or practice.

Please note: A fall sport student-athlete who arrives for preseason without all paperwork completed and submitted at medical review will not be allowed to try out or practice.

Student-athletes with diagnosed ADHD must have annual documentation of care by the treating physician.

** The NCAA now requires all NCAA student-athletes to show proof of sickle cell trait testing with a positive or negative diagnosis. Both new and returning athletes must provide this at medical clearance.

First-Year Participants

All first-year participants must come in with a physical signed by a D.O. or M.D.

In addition to the Athletics & Recreation required medical forms, you must also complete all insurance forms at:

Athletes who are required to be on campus prior to September 1st, when the university's insurance plan year begins, may choose an early arrival insurance plan for coverage in August. Additional information about the Early Arrival Plan, and instructions on how to sign up, can be found at the Student Health Insurance link above.

New students with a significant previous injury or illness that required treatment (i.e., if you were seen by a physician or your athletic trainer, had diagnostic tests performed, underwent physical therapy or surgery. This includes any current incompletely healed injury or ongoing illness.) must:

  • Submit all documentation for injuries, surgeries, illnesses etc. that required medical care. Documentation includes the following: operative report, copies of x-rays, MRI's, bone scans, doctor's office visit notes and radiology reports.
  • Submit a letter of release for full, unrestricted participation in the specific sport (s) that the student is trying out for at the University of Chicago. This letter must be written by the treating physician or surgeon.
  • Submit documentation for sickle cell trait, and use of banned stimulants for medical conditions.


Returning Varsity Athletes

Returning student with an injury or illness occurring over the summer, or ongoing from previous season, requiring medical treatment (this includes any incompletely healed injuries or current illness) must:

  • Submit all documentation for injuries, surgeries, illnesses etc. that required medical care. Documentation includes the following: operative report, copies of x-rays, MRI's, bone scans, doctor's office visit notes and radiology reports.
