CLUB COACHES AND INSTRUCTORS

Mr. Hsu has trained continuously in various martial arts since 1984. He
earned a number of 1st degree black belts in several of the Sport Club
martial arts programs.

COACHES AND INSTRUCTORS
(For Part or Full-time)
Instructors and/or coaches are defined as individuals
who volunteer or are hired by sport club organizations to teach and/or
coach students, faculty and staff, alumni. Clubs may have formally recognized
instructors or coaches to aid in the day-to-day instruction or coaching
of their club at the University of Chicago.
- The Instructor/Coaches must submit a resume with three references
(name, address, and phone) to the Sport Club outlining the qualifications
to coach or instruct students, faculty, staff, alumni, and spouses in
the area of their expertise. The coach/instructor may be a graduate
student, a full time staff member, or a non-affiliated individual.
- The instructor must signed Coach/Instructor Agreement with Club
- Sign an Assumption of Risk Waiver
- Clubs must submit a Instructor/Coach Pass request for each instructor/coach
to have access to facilities in which they instruct or coach.
- Coaches/Instructors may not serve as club officers nor administer
responsibilities of the club unless they are a student of the university.
Per IRS regulations, payments for personal services constitute reportable
and taxable income. Therefore, these payments must be made directly to
the individual or business entity through the Sport Club accounts. Examples
of independent personal services include, but are not limited to: Consultants,
Entertainers, Individuals providing coaching, instruction, seminars or
workshops)

- *Consultant and Independent Contractor Questionnaire
- A complimentary pass may be given to coaches and instructor(s) to
use for admission to the facility for approved and organized club practices.
The number of passes issued we be limited to one per club unless written
request for additional passes is approved.
- Reimbursement for Expenses-Coach/Instructor wishing to be reimbursed
for expenses on road trips must be granted permission from the club
officers and follow established policies for payment..
GUEST COACHES AND INSTRUCTORS
(For Seminars and Workshops)
Sport Club teams recognized by the Office of Intramural, Recreation,
and Sport Clubs and the Assistant Chair Intramural, Recreation, Sport
Clubs may invite a guest coach or instructor to the University of Chicago
for the club practices, lectures, and special event programs hosted by
these organizations. Guest Instructors and/or coaches are defined as individuals
brought to the campus by the organization for special class instruction
or programs.
For approval of guest instructors and coaches:
- The Club President or an elected officer shall make a request to the
Assistant Chair Intramural, Recreation, and Sport Clubs for the guest
instructor or coach at least 48 hours prior to the proposed arrival.
- The Club President must certify in writing the instructor/coaches
qualifications, via resume, and state benefit to the club.
- The guest instructor/coach must complete and sign an Assumption of
Risk Waiver.
- Sign a contractual Agreement with the sport club team for services
that will be rendered.
- If payment is a criterion of the agreement, a W-9 Tax Identification,
Certificate of Compliance form, Consultant and Independent Contractor
Questionnaire, and three (3) Service Providers names must be completed
and attached to the request for services.
- Clubs must submit a Instructor/Coach Pass request for each instructor/coach
to have access to facilities in which they instruct or coach.
- The Club President or an elected officer will receive written notification
of approval for guest instructor or coach and pick up a facility pass
from the Office of Intramural, Recreation, and Sport Clubs during the
day of the event or the Friday before 12:00pm for a weekend program.
